When Jason Fish, MD, FAAFP joined the team at First Health Care Group in Bloomington, he brought with him the principle of a patient-centered medical home. In his previous post in Alabama, Dr. Fish had been recognized for patient care, patient relations and a constant practice of putting the patient first.
The benefits of a patient-centered medical home are numerous. “We can look at the big picture,” says Cathi Barzilauskas RN, clinic administrator at First Health Care Group. An individual’s health requirements can cross into a variety of specialties for even the simplest of conditions, and First Health Care Group will, as a medical home, “coordinate all that for our patients,” says Barzilauskas. “We take the worry away from the patient so they can focus on getting better and living life.”
First Health Care Group also emphasizes routine screenings and preventive measures to catch warning signs early. “We want to save the patient invasive procedures, surgeries, and life-changing medical events,” says Barzilauskas. For example, First Health seeks to detect, treat and monitor a patient’s high cholesterol before it ever becomes a heart attack. They track the dates and results of these routine screenings and send reminders. And through First Health Care Group’s electronic records patient portal, individuals have constant access to their medical records and can track their own treatment process.
